For optimal durability, we recommend attaching it to flat, clean surfaces where it can adhere fully.
You have full control over what you'd like to display on your loved one's profile. If you don't have any videos, there's no obligation to add any.
We support YouTube embeds, meaning you can even add videos that aren't your own (e.g favourite songs) to a profile.
No! There are no monthly subscriptions or additional fees to keep the profile online. Your purchase of the Forever Story Medallion includes lifetime hosting of the digital profile. This ensures that your loved one's memories and stories are preserved and accessible for generations to come.
Each profile allows for up to 30 uploaded images. Additionally, you can embed an unlimited number of videos and write an extensive text biography without any word limit.

I purchased the QR code to sit on my son’s bench. It was easy to set up and add photos. Videos I found a little harder but got there in the end. It would be easier if they could just be uploaded straight from phone library. I love the idea that people walking past his bench will now be able to know all about the wonderful person he was and he won’t just be a name amongst a sea of other benches. Such a lovely way to honour our loved ones.

Absolutely love this for my Mum’s memorial bench. It’s a great way for anyone passing to read her story, who she was, see photos of her and for her to be remembered in a special way. It’s such an easy process to set up and apply. It’s great that there’s a ‘tribute’ section for anyone who knew her, to share memories or leave tributes to her. Overall, a brilliant idea.

Photo file a bit limited. Haven’t worked out how to get my videos onto YouTube. The concept is great. Not sure if it’s going to be worth it in the long run.
Had a little trouble registering but more me than them! Once registered I have been able to upload photos and a video, also a bio for my son. Such a wonderful way to remember him.

I have had a QR plaque on my sons stone for 16yrs but unfortunately when I came to order another plaque from the same company it was not in business anymore, so started a long search online for a company who did them finally i found this company which offered them at a good price and had very good reviews, I was very pleased with delivery which was quick and very pleased with plaques they were packaged well and were in a lovely little presentation box, setting up was very easy and explained well on a email with video attached. I would recommend this item, you can see size on included picture. 5 stars 🌟 in all areas thank you for making it so easy and straightforward.

This is a very good concept, it could be made better by allowing the owner of the medallion to move, pin, edit or delete a tribute, it would have definitely got 5 stars if this option was available

Excellent quick service , Easy to do even for me not very tech savvy , I made one mistake but the back up from foreverstory , Quickly and without fuss sorted it for me , making it all a brilliant service and now when I go to visit Dolly it’s not just a cold stone in a graveyard, If you scan the QR code you can see and here that special person in my case Dolly , I would highly recommend.

This was such an easy and straightforward experience and I was so pleased with the result for my son’s memory. He was into modern technology so this is such a fitting tribute his memory will always be alive for everyone not just us his family.

After learning about my uncle's story on Ancestry I really wanted to find a way to share it. He served in WW2 and I was able to upload his flight log and pictures of his Royal Air Force squad, along with the story of his tragic passing at 27 after surviving the war. Without this, his grave would just be a name on a stone, so I'm really grateful.

Excellent love the idea
It took me quite awhile to do this as I’ve been quite ill. However, now that I’ve done it, I think it’s absolutely wonderful. I lost my wife 3 1/2 years ago and this has given me the opportunity to show the world. What an incredible person she was. When we were married, we often used to visit cemeteries of where how past family members had been. We would look for their gravestones and find their information of births and deaths. But how wonderful it would’ve been to have these on their headstones at the time to know what life they lead, their loves , their families. In fact, their whole background. Being 81 now and having more years behind me than ahead of me, I’m actually thinking of doing my own in readiness. I think it’s a brilliant idea a very simple way of doing it with wonderful results. Thank you so much forever story for providing this.

Brilliant idea My daughter did all the tech work and said it was easy , only thing she wanted was to be able to put more photos on , 30 photos for 89 years of life was had to pick which photos . Have ordered another for my dad . Keeps my parents alive .
